This role involves providing personalized patient care in our facilities, with the opportunity to also deliver in-home care through our extensive network. Our ideal candidate is a compassionate professional with a strong commitment to recovery and wellness, who can effectively collaborate in multidisciplinary teams to ensure comprehensive patient care across all sectors. Assess patient condition by examining muscle strength, mobility, and functionality. Develop individualized plans of care consistent with the treatment goals. Implement therapeutic interventions, monitor progress, and adjust treatment as necessary. Educate patients and families about treatment plans and ongoing management strategies. Coordinate with healthcare professionals to ensure optimal patient care. Document patient care and progress thoroughly in accordance with regulatory requirements. Strong diagnostic and clinical reasoning skills. Excellent communication and interpersonal skills. Ability to manage multiple patients with varying health needs. Proficient in using physical therapy tools and technology. Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapist license in the relevant state(s). CPR and First Aid certifications preferred.
