About the Role: As a Registered Nurse (RN) in Home Health, you will play a critical role in providing direct and indirect patient care through the application of the nursing process, which includes assessment, planning, implementing, and discharge needs of the patient and his/her family. Guides and supervises assigned nursing personnel, collaborates with other health care professionals, coordinates ancillary staff, functions within the framework of the policies and procedures of MRHC, and within the legal framework of the State of Oklahoma. Minimum Qualifications: Current and valid Registered Nurse (RN) license in the state of Oklahoma. Graduate of an accredited school of nursing. Basic Life Support (BLS) certification. Proven clinical nursing experience, preferably with at least one year in home health, community health, or a related setting. Strong knowledge of nursing principles, patient assessment, and care planning. Preferred Qualifications: Experience with electronic health records (EHR) and telehealth technologies. Demonstrated experience in patient education and chronic disease management. Responsibilities: Conduct thorough patient assessments to evaluate health status, needs, and progress in the home setting. Develop and implement individualized nursing care plans based on patient assessments and physician orders. Administer medications, treatments, and therapies as prescribed, monitoring patient responses and documenting outcomes. Educate patients and their families on disease management, medication adherence, wound care, and health maintenance. Coordinate with physicians, therapists, social workers, and other healthcare professionals to ensure integrated care delivery. Maintain accurate and timely clinical documentation in compliance with regulatory standards and agency policies. Identify and report changes in patient condition promptly to the appropriate healthcare providers. Promote patient safety by adhering to infection control protocols and ensuring a safe home environment. Participate in quality improvement initiatives and continuing education to stay current with best practices in home health nursing. Skills: In this role, your clinical nursing skills will be essential for conducting comprehensive patient assessments and delivering effective care tailored to individual needs. Strong communication skills are vital for educating patients and families, as well as collaborating with multidisciplinary teams to coordinate care. Organizational skills will help you manage documentation, scheduling, and compliance with healthcare regulations efficiently. Proficiency with technology, including electronic health records and telehealth platforms, will support accurate record-keeping and remote patient monitoring. Additionally, critical thinking and problem-solving abilities will enable you to respond promptly to changes in patient conditions and adapt care plans accordingly.
