Job Description & Requirements - Specialty: Acute Care Case Management - Discipline: RN - Start Date: ASAP - Duration: 12 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Role: Lead Case Manager (Inpatient Rehabilitation) | Openings: 1 Schedule: 8:00a–4:00p | 32 hrs/week | Weekends/Call: None Unit: 36 beds | Patient Population: 18 (cardiac, ortho, neuro, debility; general IPR) EHR: Pinpoint, Rehab Connect Required Education/Licensure: Current RN or Social Work licensure; BSW/MSW (RN preferred) Preferred Certifications: Case Management/Rehab certification (e.g., CCM, ARN, ACM, ANCC RN-BC) Experience: 2 years inpatient case management/social work (acute/rehab preferred); leadership/supervisory experience strongly preferred Key Duties: Discharge planning, utilization review, family/team conferences, and coordinating resources for safe discharge Must-Have Skills: Basic computer skills; strong English communication (verbal/written); professional, cooperative, team-oriented United Staffing Solutions Job ID 983662.
