when you need it. Job Summary: Coordinates systems and services
with an organized, trans-disciplinary team approach which delivers
quality, cost-efficient care to patients through the continuum of
health care services. Collaborates with the patient, family and
other members of the health care delivery team to facilitate
appropriate movement through the care continuum. Job
Responsibilities and Requirements: PRIMARY RESPONSIBILITIES
Assesses data to determine medical necessity and appropriateness of
level of care/admission based on approved criteria. Completes
insurance verification and authorizations for specific patient
population from preadmission through discharge. Manages patient’s
benefits by checking insurance eligibility to optimize payment of
services rendered for alternative cost-effective levels of care.
Notifies physician, business office, and patient/family of
insurance denials. Attends weekly treatment team meetings with
physicians, therapists, and nursing staff to discuss treatment
options, benefits, insurance authorizations, and discharge
planning. Collects, records, and maintains clinical information
gathered from concurrent or retrospective review and assess for
medical necessity criteria. Conducts appropriateness of admission
and continued stay reviews concurrently or retrospectively. Works
effectively with internal and external customers and proactively
improves case management processes. Works in a constant state of
alertness and safe manner. Performs other duties as assigned.
EDUCATION Bachelor's degree in Mental Health or related field
EXPERIENCE Three years' experience PHYSICAL REQUIREMENTS Frequent
reaching, sitting, standing and walking. Frequent use of vision and
depth perception for distances near (20 inches or less) and far (20
feet or more). Frequent use of hearing and speech to share
information through oral communication. Ability to hear alarms,
malfunctioning machinery, etc. Frequent keyboard use/data entry.
Occasional bending, stooping, climbing, twisting and gripping.
Occasional lifting/carrying and pushing/pulling objects weighing
0-25 lbs. Occasional use of vision to identify and distinguish
colors. Occasional driving. Rare repetitive foot/leg and hand/arm
movements and squatting. REQUIRED PROFESSIONAL LICENSE AND/OR
